Having trouble getting started? - We are here to help!
- An account can have up to (2) Adults and (5) Children.
- All accounts have a 'Primary Account Holder'
- The 'Primary Account Holder' is legally responsible for the contractual guarantee of your plan enrollment. All primary account holders are also responsible for billing on their account.
- On this form you simply enter in your 'Information' to begin creating an account.
- If you wish to add any additional members please click '+ ADD ADULT' or '+ ADD KIDS'.
- Once you have entered in your information to create an account under your doctors plan, please 'AGREE' to our messaging terms and conditions.
- Once everything is 'COMPLETED' click the 'SUBMIT' button to be taken to your plans 'CONTRACT'.
NOTE: It can take up to 30 seconds to process your account and generate a custom contract for your plan.

Signing your plan contract.
- After submitting your info we will automatically populate a 'contract' for your plan.
- This will be a digitally signed document that 'is' a legally binding contract based upon U.S law.
- Please select 'START FILLING' or 'SCROLL' through the plan contract.
