Your Plans Primary Terms
DirectVision applies an $8.00 base network fee to each patient account plus an additional $1.00 charge per additional account member. This fee is to cover processing and service fees associated with your account. To apply into the DirectVision network you must be 18 years or older. Your contractual period is set to 2 valid years on your doctors vision plan. This is an auto-renewal contract with (3) notices given prior to renewal. Your benefits are provided through the eye-care provider you enrolled directly through and all benefits and terms of benefits can be used based upon their (Status) (Open-Ended) (Single-Use). You can visit DirectVision.us to learn more and request support or information about you account. Our team will contact you within 15 minutes of your enrollment. Open-ended benefits can be used as many times as needed while you are actively a member of your doctors vision plan. A single use benefit can only be used (1) time per person enrolled on the account under your membership agreement.
